Upon completion, your medical record will be released and securely sent to you. Please check your inbox and/or your junk mail for a "Secure Email Message Notice" of your processed medical record request. Click on the 'Read Your Secure Email Message" link to access your medical record. Once you receive a notification, your medical record will also be ready on your private portal page.
You will be sent to a secure link that will lead you to a 1 step verification site where you will be required to enter the password that has been set for you. Here you will be able to download the medical record directly to your computer.
